Pre-Operative Information
Before your procedure, Nashville Periodontal Group wants you to prepare for your time in the office and at home while you heal. Although you will spend a considerable time during the consultation discussing your periodontal and/or dental implant needs as it relates to your specific procedures, each doctor and team member wants to reinforce the importance of proper preparation for your surgery. As such, we developed a guide to help prepare you for your treatment. Our goal is to provide transparent communication to allow patients to focus on their health, healing, and safety. Simply put: Preparing yourself BEFORE the procedure will assist your healing AFTER the procedure!
Informed consents are documents that patients are required to review and sign giving Nashville Periodontal Group Providers permission to initiate and treat individuals. Additionally, these documents advise patients of potential risks and benefits regarding specific procedures. Before any surgical procedure, patients are required to read, sign, and date the applicable informed consent documents. In some cases, these informed consent forms must be returned to the office before a surgery can even be scheduled.
The informed consents listed below can be downloaded, reviewed, and signed at the patient’s convenience. Please contact our office if you are not sure which informed consent documents apply to your specific case.
Drs. Akin, Temlock, and West and the staff at Nashville Periodontal Group encourage patients to consult with our office and team if questions arise after reviewing the informed consents prior to any surgical procedure.
